The purpose of this study was to systematically review the literature published in the last 10 yrs that evaluated the effect of different types of exercise on high-density lipoprotein cholesterol (HDL-C) in children and adolescents. We searched PubMed, BIREME, and Web of Science from their inception in July 2019. Both randomized and non-randomized controlled trials that compared the effects of exercise and/or Control Group on HDL-C in youth were included. The meta-analysis was conducted via Review Manager v 5.3 software, using the random effect model. Twenty-five articles were selected that involved 1,199 youth. Seventeen studies conducted moderate-intensity continuous training (MICT), 9 MICT plus resistance training (RT), 12 highintensity interval training (HIIT), and 13 with a Control Group without exercise (CG). The risk of bias score was considered moderate to high (3 to 7 points out of 11). The data showed significant relative risk to increase HDL-C only in favor of the MICT Group (SMD: 0.46; IC:0.19 to 0.74; P<0.001) and a significant risk to decrease HDL-C levels were observed in the CG (SMD: -0.52; IC: -0,93 to -0.10; P=0.01), respectively.
