Despite being appropriately equipped,the exercise laboratory was not being used to its full capability. Thiswas due to a combination of factors that included inexperiencedproviders, unfamiliarity of different ergometers parameters, and pooreducation regarding indications for pediatric exercise testing.Recognizing this, the institution focused on revitalizing and scalingthe pediatric exercise stress testing program to use multipleergometers and metabolic analyzers to enhance clinically relevantand actionable data in managing congenital heart failure andchannelopathy patients. The exercise testing data were reviewedover a 24-month period looking specifically at the total number oftests performed during a given month, the percent of the testsperformed on the treadmill versus the cycle ergometer, the numberperformed as ECG-only versus a cardiopulmonary exercise test, andthe total work Relative Value Units (wRVUs) generated.Implemented programmatic changes were reviewed and assessedfor impact against the testing metrics. Our experience highlighted themost impactful factor for program growth, which was theimplementation of a dedicated exercise team. This resulted in theimprovement in standardized and thorough reports with relevantclinical data for managing patients. Lastly, there was a demonstrablefinancial value.
