Integrated Care for Patients withChronic Kidney Disease: A Literature Review. JEMonline2020;23(5):76-87. The purpose of this study was to evaluatemultiprofessional care for patients with non-dialysis chronic kidneydisease in primary care. An integrative literature review was carriedout based on searches in the PubMed, Web of Science™, Scopus,and Google Scholar databases with the descriptors: Chronic RenalFailure OR Chronic Kidney Disease OR Chronic Nephropathy ANDPrimary Health Care OR Basic Health AND Patient Care Team ORMultiprofessional Team OR Interdisciplinary Team. The articles thatwere found consisted of 15,169 of which 9 were selected forqualitative study. Three of the 9 approached the multidisciplinaryteam, while the other components of the team, such as family doctor(3) or dialogue with nephrologist (2), nutritionists (1), and physicaleducation professionals (1). The review identified that with theassistance of an active multidisciplinary team there is a betterprognosis in Kidney Disease.
