摘要:The etiological factors of pulp alterations, mainly those of biological origin, can trigger inflammatory reactions in the periradicular tissues. Despite technological advances in endodontic treatments, increasing the predictability of success, we still face cases of persistent infection. When well indicated and performed correctly, with satisfactory success rates, endodontic surgery emerges as an option for a complementary procedure to endodontic disinfection. The present study aims to report a clinical case using the concept of endodontic microsurgery in a case of persistent apical periodontitis. A 39-year-old female patient, without systemic alterations, sought the dental clinic of the Endodontics Specialization course at Faculdade So Leopoldo Mandic, Belo Horizonte Unit, complaining of painful symptoms in the region of teeth 14 and 15. From the previous history of the tooth, it was reported by the patient that she underwent endodontic retreatment at the same institution about 5 months ago due to a previous treatment that was not successful. During the clinical examination, it was possible to observe the presence of a fistula in the vestibular region between the aforementioned teeth. After clinical and radiographic evaluation, it was decided to perform endodontic microsurgery, given the previously performed retreatment. The case report was approved by the Research Ethics Committee and is registered under CAAE number: 22295119.8.0000.5374 and opinion number: 3.651.194.
