摘要：Objective: to identify the main difficulties encountered by family members/caregivers related to passing on information from the multidisciplinary team after hospital discharge. Methodology: this is an integrative literature review carried out through studies available in the Virtual Health Library, Latin American and Caribbean Health Sciences Literature and the Scientific Eletronic Library Online portal. The criteria established for inclusion in this research were: complete and original studies available for free, period of publication between the last ten years (2012 to 2021) in Portuguese. Data were analyzed descriptively. Results: through data collection carried out in the databases, nine studies were found, which addressed transitional care at hospital discharge, communication between those involved in hospital discharge and a better program at home. Conclusion: good planning in transitional care is essential to obtain a result in the continuity of treatment of patients. Communication failures are also related to adverse events, which is why different forms of communication should be used between levels of health care to reduce the risk of complications in patient care after hospital discharge, as well as ill-planned discharges. and communication failures cause anxiety and insecurity in the patient and family.
