标题:LOW TIDAL VOLUME VENTILATION Vs NO VENTILATION DURING CARDIO PULMONARY BYPASS IN PAEDIATRIC PATIENTS FOR CARDIAC SURGERIES-A COMPARATIVE PROSPECTIVE OBSERVATIONAL STUDY

摘要:Background: Cardiopulmonary bypass (CPB) is a technique in which the function of the heart and lungs during cardiac surgeries is temporarily taken over by a machine, thus maintaining the blood circulation and the oxygen content in the body of patient. Four basic functions are served by CPB: respiration, circulation, providing a blood less field, and temperature management of body. Ventilation is stopped once full flow is established on CPB. This non physiological condition ultimately leads to an array of problems in early post operative period including various pulmonary complications. Objectives: Primary: i) After separation from CPB, to compare the PO2/FiO2 ratio, Dynamic and Static Compliance of lung. Secondary i) To monitor the respiratory complications in each patients post operatively including total hours of ventilation & length of ICU stay. Methodology: Patient will be randomly allocated into two groups (C & S). Once the full flow is established on CPB, lungs will not be ventilated in Group C & a low tidal volume of 3 ml/kg body weight along with Peak End Expiratory Pressure (PEEP) of 5 cm H20 will be delivered to patients in Group S. Expected Results: We will try to prove the hypothesis that low tidal volume ventilation during cardio pulmonary bypass will result in comparatively less post operative pulmonary complications. Conclusion: Our study will show that ventilation with small amount of tidal volume with PEEP during CPB will have better lung dynamics post operatively and better oxygenation parameters in paediatric patients for cardiac surgeries with good acceptance by surgeon.
