摘要:Background: Membrane rupture occurs prematurely. PPROM plays a significant role in perinatal morbidity and mortality. The gestational age at membrane rupture is the most important factor in neonatal problems.This study aimed toevaluate the perinatal outcomes related to latency period in pregnancies with preterm premature rupture of membranes after 34 weeks of gestation. Patients and methods: 82 pregnant women enrolled in this prospective case control study, 41withrupture of membranesbeyond 34 wks. of gestation. And 41without PPROM as controls. All patients were admitted to high risk unitat Obstetrics & Gynecology Department, Zagzig University Hospitals. They were observed in labor ward for 12 hours to exclude indications for termination. Women assigned to expectant management wereobserved for vital signs and fetal heart rate was monitored every 12 hours. Fetal monitoring with cardiotocographywas done at 2 days’ interval and ultrasonography for gestational age, and AFI was done twice weekly. Results: The maternal problems were more in patients with PPROM with prolonged latency period especially infections which was 30% when latency period reached 3weeks versus 9.1% when latency at 2weeks and 0% at latency within the first week of PPROM.Fetal outcome showed fetal distress being the only significant difference between the studied groups. Fetal and neonatal problems in patients with PPROM were more with short latency period especially respiratory complications; low birth weight and hospital stays > 4 days. Conclusion: The longer the latency period, the better the neonatal prognosis, especially in terms of respiratory disorders and prematurity-related issues, but still some maternal risks especially infections.
