摘要:Migraine can be a debilitating condition that places a substantial burden on the affected individual and society. Despite significant advances in the medical management of this challenging disorder, clinical data has revealed a proportion of patients who do not respond adequately to pharmacological intervention and remain symptomatic. Recent information on the pathogenesis of migraine argues against a central vasogenic cause and substantiates a peripheral mechanism involving pinched craniofacial nerves that contribute to the generation of migraine. Thus, botulinum toxin injection is a relatively new treatment approach with demonstrated efficacy and supports a peripheral mechanism, and patients who fail optimal medical treatment and experience headache improvement after injection at specific anatomic sites may be considered fit. for subsequent surgery to decompress the entrapped peripheral nerves. Therefore, migraine surgery is an exciting prospect for appropriately selected migraine sufferers and will continue to be a thriving field filled with research opportunities.
