摘要：مقدمه: آموزش کارهای عملی پزشکی طی دو دهه اخیر در دانشکده‌های پزشکی دستخوش تحولات چشمگیری شده است و به سمت گسترش و تقویت آموزش‌ها در مراکز آموزش مهارتهای بالینی رفته است و لازم است، تأثیر این مراکز در میزان یادگیری مهارتهای بالینی توسط دانشجویان مورد بررسی قرار گیرد. هدف از این مطالعه، مقایسه یادگیری مهارتهای بالینی دو گروه از کارورزان پزشکی با آموزش سنتی (شیوه جاری) و آموزش با استفاده از مرکز آموزش مهارتهای بالینی بود. روش‌ها: این مطالعه یک مطالعه مداخله‌ای نیمه‌تجربی است که در سال‌های تحصیلی 1386 و 1387 بر روی دو گروه از کارورزان رشته پزشکی که برای گذراندن دوره آموزشی خود به بیمارستان کودکان بهرامی وابسته به دانشگاه علوم پزشکی تهران معرفی شده بودند آموزش تعداد 25 نفر از دانشجویان به روش سنتی و تعداد 19 نفر دانشجویان با استفاده از مرکز آموزش مهارتهای کودکان انجام شد. روش نمونه‌گیری در این مطالعه، نمونه‌گیری ساده غیرتصادفی بوده است. در این مطالعه، کارورزان آموزش داده شده، با روش آزمون ساختارمند عینی بالینی (Objective Structured Clinical Examination) مورد ارزیابی قرارگرفته و نتایج در نرم‌افزار آماری SPSS و به کمک تست‌های آماری Kolmogrov-Smirnov, Paired T-test و Independent T-test مورد آزمون و مقایسه قرار گرفته‌اند. نتایج: نمرات مهارتهای بالینی کاروزان پزشکی گروه تجربی و شاهد، قبل از شروع و پس از اتمام دوره تفاوت آماری معناداری داشتند، میانگین و انحراف معیار نمره مهارت بالینی دانشجویان گروه تجربی پس از آموزش 84/14±69/120 و گروه شاهد 20/11±87/102 و میانگین تفاضل نمرات قبل و بعد از آموزش در گروه‌های تجربی و شاهد به ترتیب 32/12±60/29 و 97/7±45/16 و اختلاف معنا‌دار بود (001/0P<). نتیجه‌گیری: آموزش مهارتهای بالینی در مرکز آموزش مهارتهای بالینی بیمارستان کودکان بهرامی در ارتقای عملکرد مهارتهای کودکان در کارورزان تحت بررسی بیشتر از آموزش با شیوه سنتی مؤثر بوده است. پیشنهاد می‌شود، آموزش مهارت‌ها در مرکز مهارت‌ها در خلال آموزش‌های سنتی کودکان گنجانده شود.
其他摘要：Introduction: During the recent two decades, practical medical skills’ training has experienced dramatic changes, which has led to establishment of CSLCs (Clinical Skills Learning Centers). Thus, it is necessary to evaluate the effects of these centers on students' learning of the relevant procedures. The main objective in this study was comparing the output of skills training processes in two groups of medical interns trained in conventional settings (training only on bedside) and conventional settings plus the use of CSLCs. Methods: This is a quasi experimental study conducted on two groups of medical interns introduced to Bahrami Hospital (affiliated with Tehran University of Medical Sciences) for their pediatric course. Twenty five students were taught by the conventional method (control), and 19 students received training in Pediatrics CSLC (experiment). The sampling method was simple & non-randomized. The study used OSCE (Objective Structured Clinical Examination) for assessments of student’ learning, and SPSS15 software, Kolmogrov-Smirnov, paired and independent T – tests for analytical purposes. Results: There was a significant difference between the mean scores of the two groups of interns before and after training (P<0.001).Mean and SD of the scores in the experiment group after training were 120.69±14.84, while for the control group they were 102.87±11.20. Mean and SD of score changes (difference between before and after pediatrics ward rotation) for experiment and control groups were 29.6±12.32, and 16.45±7.97, respectively, which were significantly different(p<.001) Conclusion: Clinical skills training in Bahrami's CSLC was effective in promoting medical interns' pediatric practices. We suggest that training in CSLCs be included in traditional pediatric training courses.
