摘要:Primary bone lymphoma is a rare disease and few series are reported in literature. Che- motherapy (CHT) and radiotherapy (RT) are the cornerstones of treatment for all lym- phomas. We evaluated a single Center continuative series of 21 patients to evaluate overall treatment and prognosis. According to histotype, 85.7% of patients were affected by diffuse large B-cell lymphoma. Age ranged from 19 to 82 years. The most frequently affected site was the spine (6 cases), followed by ileum, femur and mandible (3 cases each). Four patients were treated with chemotherapy alone, 17 patients underwent both chemotherapy and radiation therapy. Six patients were affected by a pathologi- cal fracture. Surgery was performed in four patients (19%), in two cases before CHT, in one case early after the beginning of CHT and in one case after CHT and RT treatment. Overall survival was 74.2% at five years. The mainstay of treatment of primary bone lymphomas remains chemotherapy com- bined to radiation therapy and surgery maintains an ancillary role and should be aimed to restore function minimizing potential delays in starting of chemotherapy. Postponing surgery after chemotherapy can allow an immediate beginning of systemic treatment and lead to a better local status at the moment of surgery
