摘要：The South Eastern Integrated Information Portal (SHIIP) is a portal-based technology solution that provides real-time feedback and summarized data to enhance and help plan care. This tool supports the Health Links program, which provides a new model for care in Ontario at the clinical level, in which all health care services in a community accessed by a patient are coordinated into a single plan. SHIIP provides the technology to enables a multi-agency care process by collecting patient demographic and clinical data from hospital databases. Some projected benefits include enhanced patient experience, reduced workflow and improvements in the efficiency of access to patient information.
