摘要：Integrating health and social care is becoming a need for most countries facing an ageing population and increase in social needs. Health and social care providers have to rethink and rearrange their care approaches to bring better quality and effectiveness. Information systems have been originally designed and developed in a separate way but they are increasingly required to work in synchronicity. Some leading regions are paving the way to health and social care integration from the information systems perspective. This workshop aims to bechmark these health and social systems through short presentations supported by live demos to show how the IT system works. We will to cover the Catalan, Scottish and Finnish case and some experience overseas from US or Australia. This will be followed by a roundtable discussion to highlight milestones and hints in achieving worldclass health and social care integration as well as detecting new areas of development.
