摘要:There are different EU research initiatives and projects that are investigating integrated care practices. These projects focus on vulnerable populations for which integrated care might be most beneficial, e.g., chronically ill, frail elderly, and multi-morbid persons. The projects have identified and described an impressive number of promising practices; several plan to evaluate these in the coming years, and others plan to facilitate the wider-spread implementation and scaling up of such programmes. During this session, five of such EU projects will present their findings, approaches, plans and connections: 1) Sustainable Integrated Care Models for Multi-Morbidity: Delivery, Financing and Performance (SELFIE1), 2) Sustainable Tailored Integrated Care for Older People in Europe (SUSTAIN2), 3) Joint Action on Chronic Diseases (Ja-CHRODIS3), 4) Advancing Care Coordination and Telehealth deployment at Scale (ACT@Scale4) and 5) Scaling Integrated Care in Context (SCIROCCO)5. The first aim is to provide a coherent overview of the academic methodologies and pragmatic approaches applied by these innovative research programs in the delivery, evaluation, scaling and sharing best practices of integrated care for vulnerable populations. The second aim is to discuss what policy, practice and science can learn from these European-wide projects.
