摘要：O artigo trata das barreiras de acesso a serviços especializados em oftalmologia, a partir do itinerário terapêutico de pacientes assistidos no Instituto Benjamin Constant (IBC), no Rio de Janeiro, Brasil. Referenciais teóricos e metodológicos das ciências sociais e da saúde apoiaram a análise, possibilitando reconhecer as dinâmicas sociais que atravessam os itinerários. Foram estratégias metodológicas: análise de documentos oficiais, levantamento da assistência no estado do Rio de Janeiro, entrevistas com pacientes e funcionários do IBC; observação participante. O estudo possibilitou identificar as seguintes características do tema: (1) adoecimento e risco; (2) itinerários percorridos; (3) recurso aos serviços privados e apoio familiar; (4) assistência pública descontinuada e não orientada pela integralidade (SUS); (5) lugar destacado das óticas. Os resultados apontam os itinerários e barreiras encontrados como uma construção social e cultural delimitada pelas condições materiais e simbólicas do grupo da pesquisa.
其他摘要：The paper deals with the barriers of access to specialized services in ophthalmology found in the therapeutic itinerary of patients from the Benjamin Constant Institute (IBC), in Rio de Janeiro, Brazil. The analysis was founded on theoretical and methodological frameworks of the social sciences and health, enabling to recognize the social dynamics that crisscross those itineraries. As methodological strategies we used analysis of official documents, surveys of healthcare in the state of Rio de Janeiro, interviews with patients and staff of IBC; participant observation. The study identified the following features: (1) illness and risk; (2) the paths that were followed; (3) the use of private services and family support; (4) discontinuities in and lack of comprehensiveness in public care (SUS); (5) the outstanding place of the points of view. The results show the itineraries and the barriers faced as a social and cultural construction bounded by material and symbolic conditions of the group under research.
