摘要：Fundamento: las fracturas de procesos transversos presentan poca frecuencia, tienen una inci- dencia no descrita y se reconocen como lesiones menores. Las radiografías simples en ocasiones son sufi- cientes para hacer el diagnóstico y la tomografía axial computarizada con reconstrucción tridimensional, es el estándar de oro para el diagnóstico. En la mayor parte de los casos el reposo es el tratamiento de elección, además de medicación para el dolor y el uso de órtesis. Objetivo: presentar un caso con fracturas de múltiples procesos transversos lumbares conti- guos. Caso clínico : paciente masculino de 58 años de edad, fumador inveterado y sedentario, con an- tecedentes de hipertensión arterial con tratamiento regular, que luego de bajar unos escalones, resbala y se cae sentado. Acude al servicio de urgencias con dolor intenso y ligero aumento de volumen a nivel de la región lumbar, con dificultad para la marcha y escoliosis antálgica. A la exploración física se constató dolor a la palpación de la fosa lumbar izquierda, maniobras de Finck y de Dejerine positivas. Dolor a la flexión y lateralización del tronco. En la tomografía axial computarizada simple de columna lumbosacra con reconstrucción tridimensional se observaron fracturas de los procesos transversos izquierdos desde la primera a la cuarta vértebras lumbares. Se decidió tratamiento conservador basado en reposo, analgési- cos y uso de una órtesis para columna lumbosacra por cuatro semanas. Conclusiones : las fracturas de los procesos transversos lumbares son poco frecuentes y en ocasiones indican lesiones graves asociadas, que pudieran pasar desapercibidas. Cuando se generan de 272 forma aislada, el tratamiento conservador con reposo, analgésicos y órtesis, ofrecen buenos resultados funcionales.
其他摘要：Background: transverse process fractures are low frequent. They have an unknown incidence and are mild lesions. Simple radiography sometimes is enough to get the diagnosis and computerized axial tomography (CAT) with tridimensional reconstruction is the gold standard. In most of the cases rest is the treatment choice, as well as medication for pain and the use of orthesis. Objective: to present a case of multiple contiguous lumbar transverse process fractures. Clinical case: a 58 - year - old male patient, heavy smoker and sedentary with a history of high blood pressure with regular treatment who slipped when he was getting downstairs and fell down the floor. He arrived at the emergency service with acute pain in lumbar region associated with difficulty to walk and antalgic scoliosis. Besides it was accompanied by mild volume increase. At physical exploration pain at palpation in left lumbar fosse was noted, Finck sing and Dejerine maneuver positive. There was pain at trunk flexion and lateralization. The rest of the physical exam was negative. The lumbosacral CAT with tridimensional reconstruction showed fractures in transverse process from the first to the fourth lumbar vertebras. Conservative treatment was indicated based on rest, pain killers and the use of an orthesis for lumbosacral spine for four weeks. Conclusions: lumbar transverse process fractures are less frequent injures that might indicate severe associated lesions that can be overlooked. When they appear isolated, the conservative treatment with rest, pain killers and the wearing of orthesis give good functional results.
