摘要:Epilepsy is one of the most common neurological diseases in the elderly and associated with a high mortality. Distinguishing epilepsy from paroxysmal non epileptic events can be a challenge for the physician. The diagnosis of epilepsy is first based on clinical examination but practitioners can be helped by simple biological tests and cerebral imaging, especially for a first-ever event. Electroencephalogram remains essential for the evaluation of seizures in the elderly patients but may show non specific changes related to aging. The presentation of epilepsy in old age is often less specific than in young adults, and partial seizures are more common than generalized ones. Metabolic abnormalities, infection, and drugs are frequent factors responsible for the occurrence of acute symptomatic seizures, so common amongst the elderly population. The international classification of epilepsy is not easily applicable in the elderly. We propose a more pragmatic geriatric model of reasoning, based on profitability, taking into account age, associated pathologies and acute factors of decompensation.
