摘要:The benefit of prostate cancer screening in reducing mortality remains controversial. The risk of excessive medical, diagnostic and therapeutic intervention is significant, reason why ethical principles of quaternary prevention should not be overlooked. A shared-decision should be in order. But for this to happen, good communication/doctor-patient relationship is critical. The case described here under addresses these and other issues, questioning the non-usefulness of the screening. The case relates to a young man with metastatic prostate adenocarcinoma who, years before, presented already an increase of prostate-specific antigen (PSA) that was not valued. A difficult patient, whose diagnosis made it even harder to create an empathic relationship between doctor and patient. This situation was successfully overcome further to a participation of the family doctor in a Balint group.
