摘要:INTRODUCTION: Electrocardiogram (EKG) is a graphic record of electrical potentials generated in the heart; produced by a device called an electrocardiograph. Standard 12-lead ECG includes 6 limb and 6 precordial leads. It represents a diagnostic tool for many cardiac disorders: myocardial ischemia; hypertrophy/dilatation of the heart cavities; rhythm disturbances; conduction abnormalities; inflammatory diseases of the heart. However, making the ECG sometimes we can obtain indirect information about non-cardiacdisorders. This refers to: electrolyte disturbances (hypo-/hyperkalemia, hypo-/hypercalcemia); the toxicity of certain drugs (digitalis, tricyclic antidepressants, i.e.); hypothermia; intracranial disorders (primarily hemorrhage); anemia. AIM: On the example from practice, to highlight the importance of making the ECG in patients who are not cardiac patients, neither they are presented with cardiac problems. MATERIALS AND METHODS: A case reports from 38-year-old man who complained of a headache, fatigue, muscle weakness and persistent vomiting back two days. CASE REPORT: Clinical examination showed completely normal neurologic findings; and also revealed BP: 240/120 on both hands. Previously, the patient was completely healthy. When the ECG was recorded we observed changes in the tracing which indicates a serious hypokalaemia. The whole medical history and clinical presentation were suspicious for renovascular cause of hypertension. The patient was transported to the hospital; where laboratory findings confirm a serious hypokalaemia of 2.1 mmol / l; which was immediately substituted parenteral. A kidney ultrasound was performed and finds stenosis of the left renal artery; so, the patient was hospitalized in nephrology unit. After preparation, renovasographywas done; and it indicated the luminal narrowing the left renal artery of 99%. In the same act lesion was opened and stented with excellent results. The patient after a brief hospitalization was fully recovered and discharged. CONCLUSION: Thanks to prehospitalECG performingto the patient with non-cardiac symptomatology we suspected serious electrolyte disorder and potential renovascular pathology.
