标题:¿Se sobrecarga a los médicos residentes? Estudio descriptivo sobre la falta de libranza de guardias de los médicos residentes y análisis de sus consecuencias docentes en la provincia de Valencia
摘要:Introduction The aim of this descriptive study is to investigate the absence of the mandatory day-off after an on-call period and its impact in the resident's medical education in the Province of Valencia. Material and methods We carried out an online survey adapted from a previous study carried out in Spain designed to evaluate different variables in relation to the mandatory rest after being on-call. Results 35% of the surveyed residents were not able to take their mandatory days-off. This figure rise to 50-60% of residents in tertiary level hospitals and up to 70% in surgical specialties. 70% of the residents claimed this was due to «Service needs related to staff shortages», 15% due to «the pressure and the fear of consequences» and 11% to «the improvement learning opportunities or lack of allocated learning time». The main factor which contributed to the inability to take mandatory days-off was the hospital and the specialty where the residence was being carried out rather than factors related to the individual will of the resident. Surgical services in tertiary-level hospital which presented with higher healthcare demand where the ones where the mandatory day-off were most frequently neglected. Published evidence shows the substantial harm caused by work overload on the physical and mental health as well as on the clinical, communication and ethical skills of residents. Conclusion A violation of the right to take a day-off after being on-call was observed in a number of residents in the Province of Valencia. This can have important deleterious consequences in their health as well as in their training. Further research in this area is necessary to improving residents’ medical education, health and wellbeing.
