摘要:Aim: To determine if the treatment and follow-up given to patients with cancer of the colon follow the 1995 guidelines of the National Federation of Centers Against Cancer. Method: 213 patients affiliated with the general scheme of French National Health Insurance (régime général de l’assurance maladie) in the Champagne-Ardenne and Lorraine regions (excluding Moselle) were enrolled in the study. They all had cancer of the colon and were exempted from making co-payments for care related to that disorder. Data was obtained directly from the concerned attending physicians. Results: The guidelines covering surgical protocols were generally followed; chemotherapy guidelines were followed in 8 patients out of 10. Three out of 4 patients had a colonoscopy during the year following the initiation of treatment. Only one-half of those patients who did not have a colonoscopy were followed by echo-doppler. The physicians’ specialties and the availability of up-to-date equipment were unrelated to the degree to which the guidelines were followed. Conclusion: The authors underscore the difficulties encountered in the promulgation and acceptance of guidelines in the medical community and the coordination of care between full-time hospital and office-based physicians. They emphasize that health-care networks could produce better coordination in delivered care.
