摘要:Aims: Our goals were to elaborate a list of radiological criteria suggesting inadequate endodontic treatment, select the most discriminating criteria for X-ray quality and to offer assistance in decision-making. Methods: After a review of the international literature, we selected twelve radiological criteria which suggested inadequate endodontic treatment. Five different examiners tested the referential using a sample of 411 endodontic treatments which were visible on retroalveolar X-ray films randomly selected from the daily flow of prior approval requests emanating from four local medical services attached to the national health fund. We used descending step-by-step logistic regression analysis for identifying the most discriminating criteria relative to the radiological quality of a given endodontic treatment. Results: The five examiners showed good intra-examiner reproducibility (0.62 < coefficient kappa < 0.91) as well as good inter-examiner reproducibility for their ratings concerning the overall X-ray quality of a given endodontic treatment (0.46 < coefficient kappa < 0.78). Logistic regression demonstrated that the radiological quality of treatments was related to the presence or absence of under-filling, of a void in the filling material, of apical extrusion of the filling material, of an apical lesion and also depended on the radiographic density of the filling material. The model’s sensitivity (94.4%) and specificity (89.2%) were high. Conclusions: Thanks to its very good sensitivity and specificity, the referential we developed should be useful in public health studies for detecting treatments presenting inadequate X-ray quality.
