It is divine to allay pain says Gallen. The goal of pain management is to reduce and eliminate pain and discomfort with minimum side effects and increase the duration of postoperative analgesia. This is a prospective randomized study of 200 cases done in Seth GS Medical College and KEM Hospital, Mumbai. The cases were divided into four groups of 50 each. Each group has received standard bupivacaine in the dose of 17.5 mg. Group 1 received only bupivacaine. Group 2 has received fentanyl; Group 3 has received midazolam and Group 4 has received buprenorphine. The statistical analysis was done and results were observed. With the present study, we can conclude that 25 µg of fentanyl along with 0.5% hyperbaric bupivacaine is a good adjuvant to achieve a longer sensory blockade, good quality motor blockade and prolongation of postoperative analgesia. In view of side effect midazolam is a preferred adjuvant. The results are comparable with norphine group. Cost wise norphine is the cheapest adjuvant with comparable results. So we can say, Norphine is best.
