In younger children and infants there are two main methods of lung isolation. The first is the
use of an appropriately sized Fogarty catheter placed under direct vision to isolate the
appropriate lobe or segment. An endotracheal tube can then be used to ventilate the unblocked
segments of both the lungs. The second method is the use of an endotracheal tube placed in one
or other main bronchus. We present a case of a 9 year old male child with tear in the right main
bronchus. In this case Fogarty’s catheter was used as a device to achieve one lung ventilation
till the tear was sutured. Endotracheal tube was placed to ventilate the left lung.
