摘要:Over the past twenty years we have witnessed significant advances in the understanding of the etiopathogenesis of vascular brain disease. The main efforts have been and still are directed towards firm establishment of basic pathophysiological mechanisms underlying brain parenchyma ischemia. This is of particular importance since ischemic brain disease (IBD) accounts for almost 80% of all vascular disorders of the brain parenchyma. In a complex therapeutic program, surgery with its various modalities has also found its place - from classic surgical revascularization methods, including carotid endarterctomy which has been performed for over 45 years (the first surgery on the carotid was performed by de Bakey in 1953 and the first carotid endarterectomy by Eastcott in 1954), to less invasive neurovascular procedures. Over the last several years there has been an increased interest in extracranial-intracranial bypass procedures as well as classic decompression craniotomies as a therapeutic measure in severe forms of IBD followed by herniation. In this context, after a comprehensive and thorough evaluation of etiological factors, pathogenesis and key disorders related to IBD, surgery with various application modalities has also found its place in certain strictly selected patient populations
