摘要:Evoked potential is an electrical response occuring in the nervous system following stimulation. Somatosensory evoked potential (SSEP) is a noninvasive study based on repetetive, submaximal electrical stumulation of sensory or mixed periphery nerves. Following stimulation, responses are registered from all levels of the nervous system, including peripheral nerves, spinal cord and the brain. SSEP of the upper extremities is routinely provoked by stimulating the median nerve, along with the ulnar nerve, while lower extremity SSEP is routinely obtained by stimulating the tibial nerve. The main criteria for an abnormal response are increased absolute latences, interpeak latencies and central conduction velocities; decreased conduction velocity; and the absence of wave forms. Abnormal findings may be seen in disorders originating at the level of the peripheral nerves, plexus, spinal roots, spinal cord, brain stem, thalamocortical projection and primary somatosensory cortex. SSEP is used in the evaluation of proximal nerve segments. In plexopathies, the conduction velocity is reduced through the plexus, but the response will depend on the lesion location and the stimulated peripheral nerve. The ulnar nerve SSEP has a limited significance in the diagnosis of neurogenic thoracic outlet syndrome (TOS). SSEP has a limited application for radiculopathies. As regards the CNS diseases, the technique is useful in the diagnosis of cervical myelopathies and demylinating diseases. It is also used for confirmation of brain death, in coma, and as a standard intraoperative monitoring procedure.
