摘要:Objective: The goal of the present study was to examine neuroradiological characteristics as-sociated with vascular dementia in a series of 463 unselected stroke patients during a one-year period. Methods: A standard stroke registry protocol was prospectively applied at admission and 3 months after stroke, including clinical, functional and cognitive assessments and CT and/or MRI exams. After a neuropsy-chological examination and an interview with the person who accompanied the patient, the diagnosis of vas-cular dementia was made according to NINDS-AIREN criteria. Results: Of the 463 patients included in the Stroke Registry, 273 (58.9% of the registry sample and 84% of survivors) were examined 3 months after stroke. Dementia was found in 71 cases (26%), 22 of these (8.05%) had demonstrated dementia before the stroke. Dementia was unrelated to stroke type (i.e. ishemic/hemorrhagic). With regard to other neuroradiological factors, patients with subcortical lacunar infarctions, leukoaraiosis, bilateral and multiple lesions were more likely to have vascular dementia. The same results were found when cases with previous dementia were ex-cluded. Conclusion: Dementia after ischemic or hemorrhagic stroke is common and is not dependent on a single factor. Several neuroradiological factors combine to exceed the critical threshold for cognitive decline.
