摘要：As one who is fairly often asked to respond to library questionnaires and surveys, may I make the following plea. In addition to the courtesy of a letter explaining the nature of the survey and the uses to which it will be put, plus a stamped addressed envelope, those carrying out surveys would help respondents immensely if they would include a second copy of the questionnaire or survey form. This second copy can be used as a work sheet or draft in the preparation of the answers and, later, serve as a record of the response that was made.
