The data from published studies indicate that the central executive, the attentional control component of working memory, is massively impaired in Alzheimer’s disease. Working memory deficits also occur in persons meeting criteria for mild cognitive impairment, prior to the first clinical signs of dementia. A number of studies also show that persons with mild cognitive impairment who suffer from a working memory deficit, in addition to episodic memory, have a poor prognosis and tend to decline more rapidly than those with only episodic memory impairment. Studies that have fractionated the executive control component of working memory have found pervasive impairment in the dementia phases of Alzheimer’s disease. Performance on tasks measuring manipulation, switching, dual tasking and inhibition is severely disturbed in those patients. The same components are impaired in patients meeting criteria for mild cognitive impairment, however, the pattern is more selective and milder overall. As a result, a number of recent studies have used modified versions of the classical working memory tasks to make them more sensitive to the first signs of the working memory dysfunction in the prodromal phase of Alzheimer’s disease. Finally, vascular burden and depression are discussed as factors that have been shown to modulate the extent of the working memory impairment.
