摘要：Este artigo traz dados de pesquisas que apontam a existência de uma heterogeneidade na constituição do campo de trabalho do acompanhamento terapêutico e de instituições de saúde mental, decorrente da influência dos discursos da psicanálise e da inserção social de pacientes psiquicamente comprometidos. Seus efeitos contra-transferenciais são discutidos do ponto de vista ético, uma vez que carreiam: o estabelecimento de uma relação na qual o profissional destaca insuficiências de seu paciente e assume um lugar de compensador das mesmas; o sentimento de o profissional nada poder fazer pelo paciente com seus instrumentos terapêuticos; rivalidade entre profissionais e familiares. Conclui apontando a importância de que tais aspectos sejam considerados em supervisão clínica.
其他摘要：This article works with data of researches that points to the existence of heterogeneity in the constitution of the field of work of the therapeutic accompaniment and mental health institutions. Such heterogeneity derives from the influence of psychoanalytic discourses and social insertion of mentally affected patients. Its counter-transference effects are discussed from the ethical point of view, since they lead to the establishment of a relationship in which the professional detects insufficiencies in his patient and tries to attend them or feels he is unable to deal with them or discovers the existence of a kind of rivalry between him and patients relatives. We conclude considering the importance of such aspects to be dealt with in the clinical supervision.
