摘要：Este artigo fala do descompasso existente entre as prerrogativas teóricas da reforma em saúde mental e a prática dos profissionais de saúde nos serviços. Inicialmente, busca contextualizar o problema mostrando como a psiquiatria consolidou-se a partir de uma compreensão biológica da loucura. Em seguida, mostra que este entendimento produziu tratamentos orientados pelo isolamento, pela dependência e pela violência, que, embora combatidos atualmente, deixaram suas marcas em pacientes, familiares e a sociedade. Por fim, sabe-se que a reforma psiquiátrica criou leis e reestruturou os serviços, mas ainda enfrenta dificuldades de instrumentalizar os profissionais em termos teóricos e práticos. É urgente, então, deixar aberto o campo de debates, pois para bem intervir, é preciso antes compreender.
其他摘要：This article discusses the disproportion between the theoretical prerogatives of the reform in mental health and the health professionals´ practice in services. Initially, it aims to place the problem showing how psychiatry has consolidated itself from a biological understanding of madness. In following, it shows that this understanding has produced treatments guided by isolation, dependence and violence, which, in spite of being currently struggled, (they) have left their marks on patients, relatives and society. At last, it's known that the psychiatrical reform has created laws and restructured the services, but it still faces difficulties at handling professionals in theoretical and practical terms. It's urgent, then, to remain open a field of debates, since for intervening well it's necessary first to understand.
