  • 摘要:Clásicamente el estudio de la apraxia estuvo limitado a los problemas en la ejecución gestual (Geschwind, 1965; Liepmann, 1908 [c.f. De Renzi, 1990]; Luria, 1977). Si bien se presentaron varias hipótesis sobre la capacidad de reconocer gestos y su alteración por lesiones cerebrales, González Rothi, Ochipa y Heilman (1991,1997) en su modelo cognitivo de las apraxias de los miembros incluyen la capacidad de reconocer gestos y proponen que la misma es mediada por un sub-componente específico, el praxicón de entrada de acciones. La alteración de este produce un cuadro caracterizado por un déficit en el reconocimiento de gestos que denominaron Agnosia de las Pantomimas. En este trabajo se presenta el estudio de un paciente con dificultades en el reconocimiento de gestos y sin dificultades en la producción gesrual lo que constituye una disociación, que reafirma la hipótesis planteada por González Rothi y colaboradores (1991, 1997) y se discute el valor de estos hallazgos en relación con el modelo antes mencionado.
  • 其他摘要:Classically the study of apraxia has been limited to gestural execution problems (Geschwind, 1965; Liepmann, 1908 [c.f. De Renzi, 1990]; Luria, 1977). Whilc several hypothesis has action for cerebral lesions, González Rothi, Ochipa and Heilman (1991, 1997) in their cognitive mod of limb praxis, inelude the ability to recognize gestures and they propose that the same one is mediated by a specitic subcomponent, the action input praxicon. The alteration of this praxicon produces a picture callcd Pantomimes Agnosia characterized by deficit in gestures recognition In this paper we present the srudy of a right, 53 years old malc with right FBC hemiparesis, right visual fidd defict, aphasia and difficulties in gestures recognition resulting from Jeft silvian isquemic stroke. A cognitive battery (Politis & Margulis, 1997) was used to assess praxis comprehension and production. This battery is based on the cognitive model of limb praxis (González Rothi, Ochipa & Heilman, 1991, 1997), and allows the assessment of every input or output process involve in praxis processing. Spared and impaired process could be discriminated and perfc: rmancedissociations could be stated. The battery comprised the tollowing tests: Audio-verbal ir¡put: assess linguistic input and its action output praxicon access trollgh the semantic system (the patient must execute 10 intransitive and 10 transitives gestures on verbal command). Object visual input: assess object visual input and its action output praxicon access (the patient must pantomime the use of 20 visually presented objects). Use objects: assess visual and tactilc inputs and its action output praxicon access (the patient must demonstrated the use of 20 actual objects). Gesture recognition: assess visual gestural input and its action input praxicon access (the patient must point to the picture of an object, between four toils, that matchs with the gesture executed by the examincr). Meaningless gestures imitation: assess perilexical rollte between visual analysis system and innervatory patterns (the patient must imitate 20 movements made by the cxaminer that are meaningless gestures). Meaningfulgestures imitation: assess visual gestural input through the action input praxicon and its action output praxicon access (the patient must imitate 10 transitive and 10 intransitive gestures made by the examiner). Object-tool matching: assess action semantics (the patient must point to the picture of an object, between four foils, that matchs with the picture of the tool presented as target). Naming by junction: assess action semantics (the patient must give the name of the object that the examiner defines by function). Function hlowledge assessment: assess action semantics (the patient must answers yes or no wether the function gave by the examincr for a visual presented object is correet or not). Complementary tests: Object naming: assess visual recognition, semantic access and oral naming ability (the patient must name 20 visually prescnted objects). Sinortimy judges: this test was cxtracted from the Spanish version of PALPA of Kay, Lesser and Colthear (in press) and the patient must determinate wether 20 pairs of words verbally presented means the same or noto The performance patient showed impaired gestures recognition due to an action input praxicon deficit. His gestural production was preserved (Use of objects and Meaningful and Meaningless gestures imitation), demonstrating that the action output praxicon was spared. It seems like the patient imitated both meaningful and meaningless gestures through the perilexical rollte, wich is spared, becausc the performance on rncaningful gestures imitation should have shown action input praxicon impairmcnt if the patient would imitated them through the Iexical routc (aetion input praxicon - scmantic system - action output praxicon)...
